A request for registration, duly filled in and an additional form that contains the issuer's statement on the characteristics of the security that is subject of registration according to the international standard for classification of financial instruments (available for download at our website www.crhovrs.org under Regulations and Forms).

Along with the request for registration and an additional for, all joint stock companies shall submit:
- decision on registration with the court register, if the issuer is subject to the obligation of registration in the court register,
- notification of the company ID no. for the issuer, issued by a competent body,
- decision on registration in the Register of Issuers with the Commission, if the issuer is subject to the obligation of registration with the Register of Issuers,
- list of holders of the securities that are subject of registration (hereinafter: list of securities holders) in electronic format prescribed by the Central Registry and in writing, certified by the issuer (the list is signed by a person authorized to represent the issuer, and each page of the list shall contain issuer's stamp) and which contains:
1) name, one parent's name and last / name of issuer,
1) name, one parent's name and last / name of issuer,
2) holder's address / registered office,
3) holder's identification number (Citizen's ID no for residents or another identification no. for non-resident physical entities / Company ID no. for residents or
another identification no. for non-residents-legal entities),
another identification no. for non-residents-legal entities),
4) number of securities owned by every holder in the concerned issue,
- decision on issue of securities, if the issuer is not a joint stock company,
- proof of payment of the fee to the Central Registry.

Closed-end joint stock company shall also enclose the following:
- constitutional document,
- Articles of Association, if one has been passed,
- decision on registration in the Registry of Issuers with the Commission, if the issuer is registered, of its own will, in the Register of Issuers.

Open-end joint stock company shall also enclose the following:
- constitutional document,
- Articles of Association, if one has been passed,
- decision on registration in the Register of Issuers with the Commission.

After receipt of the request for registration and the enclosures, the Central Registry and the issuer make a contract on registration of securities and on maintaining the issuer's account.
