The following should be enclosed to the request for change/correction of the name or last name:

certified photocopy of valid ID card or passport,

certified photocopy of excerpt from the birth/marriage register,

certified photocopy of the decision issued by the relevant body on the change of the name or last name,
-
certificate on invested vouchers or another document containing incorrect information (copy)
-
a proof of payment of 5.00 KM per order.
Note: If after performed correction of data you wish to have a proof of payment, you should make a payment of 5.00 KM (per order) into transaction account.no. 562100-80005420-15.
