The following is enclosed to the request for change/correction of personal ID no. (JMBG):

certified photocopy of valid ID card or passport, i.e. birth certificate (not older than 6 months) for a minor,

certified photocopy of a document issued by MIA, Decision on Change of JMBG/certificate on assigned JMBG,
-
certificate on invested vouchers or another document containing incorrect information
-
a proof of payment of 5.00 KM per holder.
In case of shares based on internal shares - a certified photocopy of valid ID card or passport should be accompanied with the original or certified photocopy of the certificate issued by the company that contains the information on incorrect data i.e. ID no. (generated) under which the person is recognized in the book of shareholders, and the correct data from ID card (name, father's name, last name and ID no.).
Note: If after performed correction of data you wish to have a proof of change, you should make a payment of 5.00 KM (per order) into transaction account no. 562100-80005420-15.
