The application may be submitted in person or through a proxy, on the premises of the Central Registry. The application submitted through a proxy must be accompanied by:

certified photocopy of an identification document that has a photograph on it (ID card, passport or excerpt from register of deaths - if a certificate is requested by an heir for the purpose of conducting probate proceedings of the securities holder on whose behalf the application is submitted),

special power of attorney, certified (except for starting the probate hearing, which must include a provision "for submitting of the request for issuing a certificate on ownership and its take-over.")

Proof of performed payment of the fee for issuing the certificate on ownership in the amount of 10.00 KM into the Central Registry of Securities account no. 562100-80005420-15
Please note that you will be issued the statement at your request only if the data in the Central Registry system is identical with the data in the request and in the enclosed identification document. In case of a discrepancy between those data, you should change the data in the system of the Central Registry adjusting it with the data from the documentation.
