You can submit the request at the Central Registry premises in person or through a proxy. The following should be enclosed to the request submitted through a proxy:

certified photocopy of identification document with a picture (ID card, passport) of the securities holder on whose behalf the request is submitted (for minors, a certified photocopy of birth certificate, not older than 6 months with the ID card of the applicant),

photocopy of proxy's identification document with a picture (ID card, passport),
-
notarized special power of attorney, which must contain a provision "for submission of the request for issuing statements and their collection".

payment of the fee for issuing statements in the amount of 5.00 KM to the account no. 562100-80005420-15
If you request to be delivered the statements by mail, you should submit the following:
-
certified photocopy of valid ID card of securities holder (for minors certified photocopy of birth certificate, not older than 6 months, with applicant's ID card),
-
proof of payment of the fee for issuing statement in the amount of 5.00 KM, to the account no. of the Central Registry 562100-80005420-15.
Statement issued at your request will be delivered to you as registered mail.
