• Report for the shareholder meeting is issued at the request of:
- the issuer,
- shareholder based on whose request the court ordered holding of the shareholder meeting,
- temporary representative authorized by the court to convene the shareholder meeting.
  • The request is submitted in writing, no later than within five working days prior to holding shareholder meeting, and is delivered to the Central Registry protocol or sent by mail, or by fax if it is submitted by the issuer.
  • The request shall state the following:
- type of report,
- date as of which the balance is sought (cut-off date),
- type and format of report,
- address to which the report is sent,
- name and last name of the person authorized to collect, i.e. receive the report, if sending by mail was not requested,
- name, last name and signature of applicant - physical person, or company name and registered seat and signature of authorized person and stamp if the applicant is a legal entity.
  • The following shall be enclosed to the request:
- certified copy of identification document, if the applicant is a physical entity and if the request is sent by mail or if the order submitted by a third party,
- decision of a competent body or of the court on convening shareholder meeting,
- proof of payment of the fee to the Central Registry, except for the annual general meeting of an issuer who does not have any outstanding due liabilities to the Central Registry based on annual fee.
