The request is submitted in writing, signed by a person authorized for representation and stamped by a legal entity's stamp. The request is submitted to the Central Registry counter or sent by mail.

The request is required to contain the following data,
for a legal entity:
- name and registered main place of business,
- address,
- company ID no.,
for the person authorized for representation:
- name and last name,
- ID card no. (BiH citizen), i.e. passport no. (foreign citizen),
for the person authorized to collect the statement:

The following enclosures are submitted with the request:
- power of attorney (if the order is submitted by an attorney),
- proof of payment of the fee in the amount of 10,00 KM into one of Central Registry's accounts.
