The request is submitted in person to the protocol or by mail and shall contain the following:

stated type of report,

date as of which the balance is requested,

type and class of securities to which the request refers,

proof of payment the fee,
-
notarized photocopy of applicant's ID card, and if the request is delivered directly to the Central Registry office - a photocopy, while presenting the original.
