The request shall contain the following:

the stated type of the report,

date as of which the balance is requested,
-
type and class of securities to which the request refers
-
proof of payment the fee.
If an entity submitting the request is a natural person, the request is sent by mail and the following is enclosed:

notarized photocopy of ID card of the applicant, and if the request is delivered directly to the Central Registry offices - a photocopy, while presenting the original.
