In which form is the list of securities holders delivered at registration? |

The list of holders of the securities that are subject of registration is delivered in electronic format (this electronic format is available on our website www.crhovrs.org at Forms-RS import, while the Instruction for Filling In is available on the same website) and in writing, certified by the issuer.
- name, name of one of parents, last name / name of securities holder,
- holder's address / registered office,
- holder's identification number (Citizen's Unique ID no for residents or another identification number for non-residents, physical entities / Company ID no. for residents or another identification number for non-residents-legal entities),
- number of securities owned by every holder in the concerned issue.
